Volunteer Agreement

I understand that by volunteering at Sherman Lake YMCA Outdoor Center I am required to follow their policies and procedures and act in such a manner that represents the four core values of Honesty, Caring, Respect and Responsibility. The safety and security of our participants and children are our top priority at Sherman Lake YMCA Outdoor Center. I understand it is my duty to report any suspected abuse to a supervisor or appropriate authority. I, the undersigned, also do hereby grant permission for Sherman Lake YMCA Outdoor Center to obtain a copy of my local, state, and federal Criminal History Records, including a driving record and National Sex Offender Registry Records. In special cases, I understand I may be asked to be fingerprinted to prove identity. I also attest that the information stated above is factual and correct. Additionally, I give consent for Sherman Lake YMCA Outdoor Center to run an annual Criminal History and National Sex Offender background check if I continue to be a volunteer for more than one year.
